The Workplace Safety and Insurance Board (WSIB) is a provincial agency to
oversee workplace safety and provide support for injured workers in the
unfortunate event that they are injured on the job.
The Queen Spadina Medical Centre - Chiropractic Clinic is registered with the
WSIB to provide care for workers involved in WSIB claims. With our
experience we can help you to access the care you need with minimal delay.
In most cases, there is no cost to the worker to access care in WSIB claims.
If you have been injured at work you should
complete the following steps as soon as possible:

Promptly tell your employer/supervisor that you have been
injured on the job including the type of injury, how it occurred and whether or
not you need any First Aid. At that point your employer will have contact
the WSIB to report your injury and establish a claim on your behalf.

Promptly seek out assessment/care from your health care
provider and tell them that the injury occurred at work. With the WSIB you
are able to consult your chiropractor directly without the need of a medical
referral and Dr Mueller is experienced in the completion of paperwork treatments
covered under the WSIB.

Contact the WSIB directly yourself. Once your injury
has been reported you will be assigned a claim number and a claims adjudicator.

As the worker you will be required to complete a
FORM 6 and submit it to the WSIB and your employer.
